We are now requiring that all U.S. customers have their Optometrist place
theatrical contact lens orders on their behalf.

Have Optometrist fill out form and fax to Ginni @ 707.237.2391
Payment can be taken care of in one of two ways; the optometrist can pay for them or you can give us a call and pay for them using your Visa, MC, AMEX or PayPal account. Please be advised payment will need to be received before lenses can be started.
Expect to receive a call from your Optometrist when lenses arrive at their office, this will usually take 2-3 weeks. Doctors can also request that the lenses be shipped to the customer directly.
